  • Asthma – If you have been advised by the surgery to submit an annual review of your asthma symptoms please complete our online Asthma Review form.
  • Coronary heart disease
  • Hypertension – If you have been advised by the surgery to submit your blood pressure readings on a regular basis, please complete our online Blood Pressure Review form.

Travel Vaccinations

Please visit our Travel Room for vaccinations and travel advice.

Once you have completed our online Travel Risk Assessment form, a practice nurse will then assess your form and telephone you to discuss any vaccinations which may be needed, and to make you an appointment. Please make sure we have an up to date contact telephone number for you, by using our online Change Personal Details form.

If you have a record of previous immunisations please submit this with your travel form. If you were born overseas, have no record of previous immunisations, or missed out on childhood immunisations, then you could need more vaccines than you think to ensure that you are covered.

Please note:

• The vaccines are not a stock item and have to be ordered specifically for you.
• Some vaccinations and all anti malarial medications are not available from the NHS, and there will be a charge for these items.
• Consultations required for the administration of Non-NHS treatment will incur an additional cost.
• You will be asked to pay in advance by cash or card for any vaccines you require. The nurse will discuss this with you before booking your appointment.
• Vaccine stock availability, and Non-NHS vaccine prices, are subject to change at any time.

Once the practice has received your payment in full, the vaccines will be ordered and a second appointment will be booked with the nurse for the vaccine to be administered. The second appointment needs to be at least three weeks before your travel, as most vaccines need 3 weeks to build up your immunity and some vaccines need to be given as a part of a course over a longer period of time.
